I think it is becoming increasing clear that the Columbia was likely doomed from the time it lifted off. The images of something hitting the underside of the left wing is telling. The subsequent failure of the left wing on re-entry points sharply to damage in the insulation system. There was no way to fix damage if indeed damage was viewed. Reports have said that they crew checked for damage, but did not do an extra-vehicle excursion. I'm not sure if they were equipped to do so.
Even if they did, there was no way to repair the damage, and no way for this shuttle to dock with the space station if that was deemed an option. JMHO> larry
