Totally anecdotal, but I heard from somebody that they heard from somebody that the cause of the AWS outage was that somebody left logging on when they pushed a deployment. So not totally a bug maybe but operator error?? Not sure. I'm sure Amazon will never explain fully what happened...none of these big software companies seem to fully own up to what happened. It probably has to be leaked.
