I asked that exact same question awhile ago. This is the helpful response I received.

However grape juice is a more potent thinner of blood without the hazard to the peripheral vascular system which the drying out of platelets by alcohol represents. If one takes alcohol in any amount one must increase intake Vitamin C, Vitamin B6 and B1 and b12 to smooth out the platelets and prevent stroke. As well it would be a darned good idea to take milk thistle and n-acetyl cysteine. If one is diabetic only take NAC with a doctor's advice.

What you get from the grape alone however is pro-anthocyandin and anthocyanin, which aids the integrity of the vascular system as well.
