The only definitive test for PCa is a Gleason score from a biopsy. But there are better ways to do biopsies than blind 12-needle biopsies, which is what most urologists will recommend. The danger of them is that they can completely miss the problem area, and then the patient remains on active surveillance while the tumor continues to grow undetected.

The 3T mp MRI should be the first tool used for serious diagnosis since, if nothing is detected, it is completely uninvasive (i.e. no biopsy is necessary). MRIs should become a standard just like mammograms are for women (prostate tissue and cancer are very similar to breast tissue and cancer). Fortunately most PCa is slow growing while within the prostate capsule so early detection offers many alternative treatments to radical prostatectomy. The longer PCa remains undetected, the fewer the options, not to mention allowing PCa to escape the prostate capsule can lead to metastatic cancers elsewhere.
