The voice quality of an iPhone is as good as GSM and the other person's cell phone. I get high quality "clear as a bell" calls from some people and utter crap from others. I use a noise canceling Jawbone Bluetooth earpiece and get no complaints from anyone I talk to with my iPhone - unless it's really windy. I only wish some of the other's I talk to had noise canceling earpieces.

Of course if I or the person I'm talking to is in a "hole" in the network, all bets are off.
