You know what they say- it's really really addictive- and that's why even very ill people don't (or probably can't) quit. There are people smoking from their tracheotomy holes- after they've had surgery on their throats for smoking related cancers. Because it IS so hard to quit, we ought to be trying really hard to prevent young people from starting- we aren't, of course, but we ought to be. For people who are concerned about sparing people pain, and preserving life, there probably aren't too many causes better than limiting smoking (although combatting obesity, and helping people improve their diets is another cause that comes to mind, when I think about how to alleviate the greatest amount of suffering, and improve and extend people's lives.)
