If you write an iPad app, you can design your own keyboard for that app. Unfortunately, that's the only app that can use it. I don't think iOS 4.2 allows you to design a new system keyboard for replacing the iOS built-in keyboard so that it could be used in any other app. It would be nice if you could do that though.
