They could make it 1000 pages. One a criminal develops a formula for being approved it will be for sale on the street for not very much.

Logical claims evaluation before the funds are distributed should be required. Every new provider should be audited at 100% for a period of time.

You are right about this -- the fraud could be stopped if they just took a rational look at new providers. Or if they simply had a phone number where reported fraud was followed up on.
