The 'old' HMO's - Harvard and Kaiser did do Preventative Maintenance. Some of the new ones pay lip service to the concept.

Word to the wise - avoid capitation systems. I did some actuarial analysis on one once - and as good as the theory is on paper - it sucks in practice. We actually found some doc's who used preventative checkups to discover 'issues' and promptly transferred sick patients (to avoid costs).

I personally use UHC's PPO - 100% in plan, 80% out of plan with catastrophic maximums. Never had an issue, but its twice as much as the HMO and other offerings I get.
