It's my take on McMillan that he feels you have to play it as
if you'll be called out on expiration. He goes into an explanation
about how arbitrage buys up big blocks if there's 1/4 points in
the difference between option price and underlying stock price.

My instinct would tell me to agree, but if you're in the money
and you're not called, it's pure gravy train time.
