The conversations are supposed to sync, but I have noticed that they don't always.

It's been erratic and unpredictable for me too. I get the impression that how messages should sync between devices is still not well defined. Should a message be duplicated and shown across all synced devices, or only selected ones? What should be the default device in the event of an incoming message? There's no way for us to specify these parameters. Hopefully, iOS 6 will sort this out. It's also unclear when a message travels via SMS or iMessage. For example, when a buddy of mine sends me a message through his WiFi network, it sometimes arrives via SMS and at other times iMessage although I'm on WiFi at all times.

a message will come in on my iPhone, and I then go to my Mac and send a reply

At least your message arrives in both devices. In my case, the message arrives in only one device, and I can never predict which one, which can be a real problem.

I'm afraid I haven't been well enough to dig into why that might be

I'm sorry to hear you're feeling poorly. Get well soon. Think Apple thoughts :-)
