I am not clear on the answers to your questions, unfortunately. I believe that they are all deaths, and that they are all a result of attacks, directly and indirectly, on Serbia. The reason for my surmise is the use of Serbian government figures. Since the government would have an interest in counting all casualties, and particularly fatalities, that could be attributed to NATO, I doubt they would limit themselves to direct bombing. I would guess that the Kosovo situation was too much of a mess to assess civilian damage within the province. If I decide to nose around further, and come up with anything more detailed, I will, of course, let you know. (I have never heard of a "broken pipe".....)
