I have seen depressions this deep. The anxiety is riveting and carries a "no return" quality that defies description. The kind of doc who is good to know if this happens is a doc who has a good track record with bipolar people. Both the manic and depressive ends of the cycle are often marked byaintense anxitey combined with coherent but delusional ideation. Spouse&I have friends with various levels of this condition, so it's a known demon. Authorities who haven't spent time with bipolar patients (and I wonder if severe postpartum depression belongs in this continuum: the triggers, stress and corticoid imbalance, surely match up) are convinced they're dealing with sudden severe schizophrenia. Different condition, different outcome, different treatment.
If there are long-term ill effects, another future depressive crisis needs to be considered. Each case responds to a different mix of meds. That's the real tragedy of limited HMO formularies.
