The thing is that not even referrals from optometrist and the good will of the laser surgeon will allow a small operator to compete against the economies of scale of a chain operator. Is is like a Ma & Pa hardware store going up against Walmart. They can hold out for awhile, but their cost eventually will be prohibitive if the chain does proper marketing in the area. Also, remember that the bulk of the procedures are being done by LVC's already, not individual doctors. They only have to improve on the marketing models.
As to the hit from a mistake, there already has been errors that have been reported. I guess that is the price you pay for being in this business. The question to you is who do you think can better take the hit from bad publicity --- a large chain doing thousands of procedures a day or a doctor who is faced with a malpractice suit?
