Who decides what healthcare procedures will be used per individual? That is quite simple. They gather a group of individuals to put together a protocol, so no one person can be blamed. If a doctor requests an exception a Star Chamber reviews the request. The Star Chamber is comprised of celebrities who played doctors on TV, doctors who decided they want a sweet government job and politicians spouses whose party is in power. That way it is always certain that the right people will get the procedures they need and others will not be allowed to burden the system.
