I would hate to ever be called upon to own up to my own aviation errors, but I still recall doing a thorough pre-flight check of my plane, only to take off with the cowl nostril plugs still firmly in place..... bright yellow with the long red streamer fluttering delightfully in the prop wash. Luckily, the cylinder head temps didn't go off scale before I circled back and landed. The tower enquired as to whether anything was wrong, to which I responded very laconically, "no, just want to check something". Their response was classic.... 'you mean like those big yellow things with the long red streamer?" (g)

I also know a guy who took off (somehow) towing a 75 pound concrete tie-down block, firmly attached to his tail. He reported to the tower that he was coming back in to land as "the plane isn't handling properly". He made it back down (somehow) without incident.

Frequently, the gods of flight are not so forgiving of such dumb acts.
