Q&A - I have probably missed the answer to this. It would seem to me that small LDL would be much more likely to get outside the lumen and build up. Therefore causing the "Glagov Effect" when a stress test is done.

Great question. However, I don't know the answer.

It would indeed be very tempting to relate various lipoprotein patterns to the degree of Glagov effect. Not an easy study to do, since it would likely require intracoronary ultrasound.

Development of the Glagov effect likely requires elaboration of destructive enzymes, e.g., matrix metalloproteinase and other elastases, enzymes that degrade structural tissue. Small LDL does indeed tend to be a more potent provocateur.
