Here, let me lend a hand on this one. It could be because many waiters and waitesses themselves smoke. Most of my acquaintance in that profession do smoke. It could be because of the charbroiling and other cooking activities prevalent in most restaurants. It could be due to genetic predisposition.

Lots of people who have never smoked and never been exposed to smoke still get cancer and heart disease. People who have never consumed alcohol get cirrhosis of the liver and other ailments commonly thought to be related to alcohol consumption. Laypeople and medical people who hold up these "scientific" studies and preach them as medical gospel are ever astounded and dismayed to learn this, and they do not like to hear it.
