IMHO, HIV/AIDS patients should only be under the primary care of an internist, who has had training in HIV and infectious disease. This is a very complex disease that can involve the nervous/cardiac/pulmonary/renal, etc., systems with complications involving infections and malignancies. "HIV wasting syndrome" is very difficult to clinically assess and manage. Treatment has become more complex and the doctor must have a very thorough knowledge on current treatment modalitiesy.
