Maybe you're right that it (poppy consumption) doesn't add up to the measured harm from legal ones, alcohol and tobacco. But if so, the reason could be that the former is illegal and therefore spurned by great numbers who would otherwise use it, and the latter two are legal and (relatively) socially acceptable.

One might look at China's extensive experience with opium.
