The PI perspective dated March 1997 is excellent reading. Page 15
Protease Inhibitors and beyond: RT Inhibitors, ZincFinger Inhibitors,
Fusion Inhibitors, Integrase Inhibitors, but no mention of Kinase Inhibitors. These folks are begging for help. Page 18 covers Medicaid. Medicaid is coverage for roughly half of those living with AIDS and 90% of children. I don't think Medicaid will cover non prescription items. Therefore, likely half the market is gone without FDA approval. Conversely, it would appear that the Medicaid people may be willing to put some pressure on FDA because of lower projected cost and the pressure on the budget.

In conjunction with maximizing sales and profits as rapidly as possible it would seem that the company should:
(1) Make Project Inform aware of the status of the program and work with them in dissemination information. Get their input on distribution.
(2) Work toward FDA approval using the self interest of each group that could bring pressure on FDA. This assumes that there is at least minimum valid data supporting the company's position. Act Up gets a lot of press. Just think of their reaction to FDA's suppressing an inexpensive medication that almost anyone can afford. Medicare needs a less expensive solution. Every one has an ax to grind. Maybe there is a way to make FDA look like a hero by expediously approving the product.

I am not usually this devious but there comes a time when you must use every edge you can get without violating your ethics.

The afore stated publication is recommended reading for anyone interested in the status of HIV treatment, politically and clinically.
