I think an important point goes missed here. And that is the payments from Medicare and Medicaid are pathetic. The reimbursements that give these physicians the profits they earn are from private insurance companies, which are effectively subsidizing the government programs, which is the root cause of the ever increasing cost of health care.

If there are no private insurance companies, which IMO is the goal of Obamacare, these guys would be earning a fraction of what they do and the result is that fewer would enter the marketplace and the services would be harder to come by, and of a lesser quality.
