If they were attacking with reckless disregard to civilian casualties than the death toll would be considerably greater. If they where trying to kill as many civilians as they could the death toll would be orders of magnitude larger.

Assuming there is any reality behind that figure I would take it to really be something more like "at least 103 people, 4 of whoom have been identified as being Hezbollah militants, have been killed." I doubt that those 4 are anywhere close to all of the Hezbollah deaths even out of those 103. I also imagine that there are Hezbollah deaths that are not recorded in that 103 figure.

Also as Lane3 said "There's a lot of space between aiming at busses full of evacuating families and guaranteeing that you never hit one." To which I would add there is even a lot of space between attacking with reckless disregard to destroying busses packed with civilians, and guaranteeing that you never hit one.
