Well, the "entrenched bureaucracy with the power to deny coverage for superfluous reasons" would not be driven by the profit motive. Right now, it's a clear benefit to the insurance company to decline, in fact some pay commission t their adjusters to to just that.

Please enlighten me the mechanism by which entrenched bureaucracy with the power to deny coverage for cost containment reasons would be avoided in this branch of government.

Vote them out of office.
