Khalil, while I don't understand accounting for this industry very well, I wonder if maybe the "net commissions and fees earned" line under revenue is offset in large part by the "Commissions" category under "expenses".

After you subtract expenses, you get an EBITDA margin of only 10% of gross written premiums. Net margin is only 3% of gross written premiums.
