Trying to answer as short as possible:

2)Yes, they call it HAART (Highly Active Antiretroviral Therapy), but
John Mellors MD, said it would very soon be called Fairly Active
Antiretroviral Therapy (FAART)
3)Yes
4)Depending on the patients, cd4 every ~3 months, viral load once a
month until undetectable, then every 3 months. Adjust therapy based
on clinical response, specially rises on HIV RNA.
