Some clarification: I virtually never use Niaspan, but over-the-counter Sloniacin or Enduracin, both virtually identical in their time-release properties to Niaspan.

Also, we almost never use niacin any more except for people with lipoprotein(a) who fail to respond to initial efforts: high-dose fish oil, high-fat diet free of grains and sugars, and thyroid normalization including T3 efforts.

That all said, if we combine ALL the studies performed on niacin, we have a mixed bag of beneficial and adverse effects. It remains unclear whether there is benefit and what precise subsets obtain benefit: Lp(a) vs. apo E4 vs. other variants. But let me be clear: Following the diet approach articulated here, combined with efforts such as vitamin D restoration, omega-3 fatty acids, thyroid management, and a full pro- and prebiotic approach to bowel flora, a minority need even consider niacin. MOST PEOPLE HERE SHOULD NOT BE TAKING NIACIN IN ANY FORM at therapeutic doses, in my view. This is a change from several years ago, but our diet and adjunctive approaches (especially bowel flora and thyroid management) were not as vigorous.
