Russ - I am a non-registered shareholder (which I expect is the case for most people) and I received the materials you described from my broker, which is the usual practice. In my case, there is sheet on which my name and address appears, which showed through a window in the mailing envelope. But this sheet has other material on it having to do with voting the shares, with boxes in which to place an "X" in accordance with the shareholders wishes. So that is the proxy card. If you use a Canadian broker, you may have received the same type of document (hope you didn't throw it out).

Yes, by all means street name holders can vote, and the material I received enables me to do so. I don't know the answer to your question about the SEDAR card.

BTW, are you sure you received the material from SJD and not your broker? My broker's name, my account number and number of shares held appears on the dual purpose mailer sheet that I described, so I know it came from my broker.
