States do something like this now, though it's limited. CT offers coverage to kids and low-income people. Premiums are not high and based on income, but it can be hard to find a doctor who'll accept the insurance and very hard to find a specialist. OTOH, it's better than nothing, and in fact the only insurance available to my son. Because of a genetic condition, he's not insurable on the open market.
