Some I can understand that circumstances make it difficult for them to move, but every person's situation is unique like that. One "ultimate" example of that is Key West, Florida. The cost of housing is outrageous, you probably can't rent a closet for less than $2500 a month. At the same time it's a tourist destination that requires lots and lots of service workers... and it's an island so it ain't like you can commute from the 'burbs. re: It's why I've seen another classification being tossed around as part of the conventional wisdom: "upper middle class." The term upper middle class has been around since I was a kid... at least.
