Darin, NXTL had a similiar problem here in San Diego county. And as fate would have it, the blind spot was in the busiest spot in the county - near the I805 and I5 choke point. Fortunately that's history now. It's my understanding that in each local market NXTL has someone drive the entire metro area each week, looking for and noting blind and/or weak spots. In addition, they keep track of customer complaints and followup to make sure the problems are fixed.

On the cell site issue, at last June's annual meeting Akerson said that the company had over 3000 (3300 if memory serves me right) cell sites planned, most of which had sites under lease and local government approval in hand.

While I'm sure that other digital companies are doing similiar things, it's good to know that NXTL is on top of things.
