Not a word about it because its not relevant to the debate. No one has said that governments and other institutions besides drug companies don't do and/or pay for a lot of basic medical research.

What they don't due is develop a lot of expensive drugs, and then pay the costs to get them approved in the US.

Sure such research does get done in other countries. By their drug companies, who do it in order to get the high profits available to successful drugs in the US (the high profits being the incentive not only to pay the development and testing cost for that drug, and to deal with all the delays, but also to pay for all the drugs that don't make it through to approval, the drugs get approved and then aren't very successful, and the drugs that make it through, sell a lot, and than are found to cause harm leading to multi-billion dollar lawsuits.)
