So someone should never have to make that choice no matter how expensive the drug or treatment?

If your response is something like "make it cheaper", then consider "how expensive" to be the resources, development costs and such, to develop, test, produce, market, distribute, and apply the drug, or treatment service or medical device, rather then the specific price tag, reducing the latter doesn't reduce the former.
