Peter: There is an article in Internal Medicine News that is interesting. Dr. Christopher Filippi, a neuroradiologist from Albert Einstein College of Medicine, reported that protease inhibitors reverse AIDS related dementia. This is an unexpected finding. MRI scans showed regression of white matter disease in patients on PIs with AIDS related encephalopathy. Sixteen patients at Yale New Haven Hospital were studied, nine on PIs (IND, SAQ, and RTV) and seven controls. Those treated with PIs showed both clinical neurocognitive improvement and improved or stabilized MRI scans showing regression of white matter disease. The seven controls showed progression of disease and were on various other non PI drugs. The thinking had been that the CNS might be a reservoir of infection that would show disease progression because of concern about PIs being ineffective in the CNS. This is an optimistic report.
