1.$19.95 flat rate might help AOL but certainly not much.
Because all online services and ISPs offer flat rate, too.
2.28.8k, 33.6k, and 57.6k are not fast enough for the use in the future on internet. (Audio,Vedio,...). Cable, ADSL will be coming soon. If AOL put lots fo money to upgarde to 57.6K, they might not get their investment back when they have to upgrade again.
3.As I know only 20% or fewer of AOL's subscribers use AOL over 20hrs now. Yes, the revenue will increase I guess but not dramatically.
4. How are ISPs in Japan?
5. I agree with your point 2 and 3.
6. The problem of AOL is how to break even and make profit. Even they break even, how much can they earn you think?
