Friendly fire
or
<<blamed by U.S. and Canadian inquiries>>

The two US airforce pilots who face disciplinary hearings on January 13 for a friendly fire incident involving Canadian PPCLI troops in Afghanistan may receive sentences of up to 64 years for involuntary manslaughter. Both the United States and the Canadian inquires into the incident have reached similar conclusions. The use of speed or go pills was not a factor in the attack as suggested by ABC, both enquiries agree on this, the pilots are to blame, both enquiries agree on this, enquiries also concur that there were deficiencies in the command structure.
In any case, the question will be resolved in a few weeks, the similar findings on both sides do not bade well for the pilots involved. Here is an article that details some facts and some further allegations about the incident.
