I read an piece on that just in the last couple of days in the context of the plan for Federal employees. There was a question about why rates didn't go down once they were under Medicare given that Medicare employees would cost the second payer less. According to OPM, they don't.

The piece wasn't clear about what they were comparing. I'm assuming that the cost does go down when you compare the cost of a 65 year old against the cost of a 64 year old and what they meant was that, as a comparison with all those covered, who are mostly young employed and their families, the Medicare group doesn't cost less.
