SI implements major changes...

...without adequate (or any?) testing or QA
...in the middle of a week
...without any prior announcement other than a vague "scheduled downtime" notice.

And when it's realized there are major problems, whatever you do, make sure not to issue any sort of courtesy "we're having troubles, hang tight" PM to users; no, let them figure out on their own that things are thoroughly effed up.

Agree with previous posters that heads - in more than just the programming department - should roll over this.
