The 40 or 45M who don't have insurance largely choose not to have it, preferring instead to take their chances.

This is absolutism inode style ... 45M people who don't have HI ALL chose not to have it....of course that's non sense. Furthermore, the ones who do chose not have it are irresponsibly burdening the system, partly because they go on crisis care only and partly because they pay nothing into it....which puts the cost on the rest of us. We've had this silly debate....i don't wish to go through it all over again.
