I think you're failing to take into account the fact that in combat, few rounds are so accurately placed. The DC sniper is shooting people much more accurately than would happen in typical combat. That's because his victims are standing around in the open. In combat, people hide behind stuff, and run as fast as they can when they are in the open. They're very hard to hit.

If you look at the cross sectional area of a human, you'll notice that the arms and legs take up a lot of area. Those people who get shot in the arm or leg by these small calibre rounds usually survive. Larger rounds have more of a shock effect.
