Absolutely it is. I can name plenty. I don't believe that insurers generally are out to get providers.

But there are some who will use every tactic to stonewall.

United Health Care, on receipt of an out of network claim, normally will wait for a month before doing anything. Then, they'll send a letter that says words to the effect of -- "You have billed us $300. If you will accept 60%, or $180 as payment in full for this claim, we can pay it within ten days. If you do not accept this discounted payment arrangement, please fax this form back and your claim will be processed within 90 days".

In fairness, it is the state board of insurance that allows them to get away with this kind of crap.
