The only thing I can say is that to the extent any of these expenditures contain a "fixed cost margin contribution", taking these expenditures out of the American health care system simply increases costs elsewhere.

And the only think I can say is that you seem to be focused on the immediate effects and not accounting for adjustments over time as the system normalizes itself in response to the decrease in certain types of business.

Every change has immediate negative effects. If you put a computerized system into a doctor's office there is extra effort and cost required for a while until things settle down, everyone is trained and proficient in its use, and the old paper filing system is no longer needs to be tended. But you appropriately measure the cost effectiveness of the change based on the end result, not based on the costly disruptive period. You are measuring the effect of Costa Rica on the disruptive period where you have CT scanners sitting idle. I'm looking at the end result--the time after the facilities have adjusted.
