Soon America may be employing third world doctors as the supply of Americans dwindles due to falling incomes.
One problem I foresee is the shortage of visa for professionals that has not expanded since the sixties (except briefly in the eighties but it returned to earlier levels). Last year the quota was met within days of the beginning of the year.

Rationing of healthcare is in our future.
