The co-pays tend to fluctuate, depending on the procedure or service.

Most policies have something called a stop gap, which puts a lid on the co-pays for the year.

Where it gets confusing is the amount allocated for the procedure or service by the carrier.

It's not a percentage of the charge, but a percentage of the reasonable and customary charges. All insurers have detailed codes used by the providers to place the claims.

I defended a health insurer in bad faith litigation for over 12 years. I have been down this trail.
