Dew, in response to your Yahoo post on visual outcomes c Wet Mac Degen. Some wet Mac Degen. pts start of with 20/20 vision though a minority. Most start off with 20/25 to 20/50 which would be considered useful vision since in most states you need 20/50 to pass a drivers license.Some start of with worse than 20/50. The main variables are how quickly they come in when they notice symptoms, the extent of having decreased vision due to underlying dry MD which is not reversible, and how close to the center of the macula the vessels are.
When the wet MD is present the abnormal neovascular vessels leak fluid which in some patients will lift the macula away from its blood supply and cause a decrease in vision which is reversible if you success fully treat the Subretinal NV. Untreated most pts have the vessels hemmorhage with the resulting disciform scar causing 20/200 vision or worse ie legal blindness.
One point I'd like to make is that anyone who has had WMD (including my mother who is legally blind from it) would be happy to just stop the progression and still have some ability to function normally -read with magnifier, recognise faces and see to drive etc. So even if there weren't any patients who had improved vision, Stable vision is an Excellent improvement to the natural course of the disease.
Another point would be that only some patients would be expected to have a significant improvement in vision with successful treatment- Those with Subretinal fluid from the subretinal nets who don't have a tremendous amount of dry changes. Another class of patients who would be successfully treated without an improvement in vision would be those whose subretinal nets were at the periphery of the macula such that they didn't start off with much decrease in vision. These patients are presently treated with laser. The resulting scar not being in the center of the macula doesn't cause as much visual impairment as the fully progressed disciform scar. It will be interesting to see if Evizon were to replace laser RX in these cases.
