There was a critical mismatch in doctrine between NATO and Warsaw that only became apparent after the Cold War ended. The US doctrine called for limited staged escalation of nuclear exchanges, whereas we now know the Warsaw Pact plan was scorched Earth total war with massive nuclear bombardment of Western Europe ahead of Warsaw Pact armies.

That would have been a disastrous mismatch in understanding our enemies.
