May be. Don't know.

Don't know if the hotbox alarm notification system actually worked.

If the news report I posted is true, then the train ran for at least 20 miles with some sort of axle malfunction generating astronomical amounts of structure weakening and fire making heat before the accident happened.

Could be a result of poor company policy leading to bad procedures. Even if that is the case, following the principle that all shit flows downhill, it might be "operator error" anyway.
