The article was about government initiatives to allow Medicaid to pay for protease inhibitors and rti's earlier in the course of an AIDS infection. There is a sizable body of opinion that the new combination drug regimens will save total hospital/medical costs if used early.

Unfortunately, current Medicaid regulations require a person to be disabled (unable to engage in substantial gainful activity) before Medicaid will pay for treatment. Thus, the Feds are talking about grating waivers to States for demonstration projects in order to provide combo therapy before the disease has done its damage. If drugs are started earlier, careers will be prolonged, more taxes will be collected, and total treatment costs will be reduced.
