You'd be surprised. Publishers of books evidently hate 'em; they complain that readers find them "intimidating". Nearly all publishers are aiming their lists at as wide an audience as possible now, and it seems they want everyone to emulate Simon Schama. Even the academic press isn't entirely opposed, and they're guilty of other crimes as well. Take Yale, for example: I don't know how it is in other disciplines, but if you're an art historian and they agree to publish you, it is your responsibility to obtain and pay for all photo rights. This could cost you as much as ten to fifteen thousand, and making arrangements for rights is, believe me, a via crucis.

Here in Italy the situation is somewhat better, for now at least. Not that you're ever paid a significant sum for your trouble, but with luck you won't end up out of pocket.
