Even now, there are two different specialists that I go to who seem to rent pretty substantial looking offices in one of the major hospitals in the world and operate with what appears to be one administrative assistant each.

Of course these doctors seem to generate a lot more than $250,000 a year. But, the administration assistants do seem to be very busy. They are (no kidding) almost always on the phone with someone from an insurance company asking for payment.

Depends on the specialty, but there aren't many like that. For example, I know of psychiatrists and hospital radiologists who could do this. But if you're seeing 20 pts/d in FP or other specialties, it would be very tough today to do that with only one or two other salaries.

Not saying it couldn't be done, but that guy isn't going to have a "light" workload and he sure as hell isn't going to be well-compensated for the effort he put into becoming a physician. For that, he'd probably choose to go to work on salary for someone else and leave the budget to his employer.

For example, I know a FP doc who chooses to see about 10-15 pts/d, and he has two admin staff plus a nurse. I think one really good admin staff could do the work, but she'd be busy -- you'd still have to have the nurse. I doubt he collects 250K, though.
